Please click the button below, print and fill out the form, and then drop it off at the parish office at your convenience. This may take 5-10 minutes of your time. There are information sections for up to 4 adults (head-of-household and spouse, dependent grandparents, dependent adult children, etc.) living at the same household address. Independent children over 21 living at this household address should fill out their own form. Below the adult fields are 4 fields for children with an additional box, if needed. If the fields for children are not applicable, please leave them blank.
